YOUNG RYLA Paperwork Checklist and Fax Cover Pages!

The registrar spends countless hours poring over 360 of these (2,500 pages), so it’s much appreciated if you verify that everything is complete BEFORE faxing! This checklist helps you do that.

 

Put a checkmark in EVERY checkbox as you verify the items. THESE completed pages are THE COVER PAGES for your fax. Please send each Applicant’s paperwork in a SEPARATE fax transmission! (hang up after one applicant’s paperwork is sent and begin a new fax transmission for the next applicant – necessary for the automated sorting on our end).

 

If anything is missing, please DO NOT FAX partial paperwork! Wait until you have ALL of that applicant’s paperwork (you’ve put a checkmark in EVERY checkbox). PRINT your info below:

  • Print this checklist, fill in the checkmarks, and make this the FIRST page of your fax transmission, followed by ALL paperwork listed below.
  • ONLY fax paperwork for FINALIZED applicants. Go to www.rmryla.org and mark them as Finalized right now if you need to!
  • ALTERNATES - collect and hold (do not yet fax) paperwork for alternates.
  • Verify that no contact information is hand-written on the application.
  • All signatures and other writing should be in BLACK ink.
  • Please send EACH applicant in a separate fax transmission.


Please fax these pages IN THE ORDER BELOW. Please DO NOT fax any paperwork before taking extra care to include the Medical Checkup signed by a doctor or nurse!

A) This page (with all checkboxes checked) followed by additional pages in this order:

 

B) The first page(s) of the printed application, showing name, address and e-mail.

      Page(s) with parent name(s) and phone numbers.

      Grade Next Fall – must be an upcoming 8th grader.

      Ensure that their birthdate seems reasonable for 8th grade.

      Page(s) with parent AND applicant signatures.

 

C) Completed and signed Ponderosa Registration form:

      Individuals NOT authorized ________________________________________

      ALLERGIES ç circle here if any severe allergies.

      BRINGING MEDICATIONS ç circle here if bringing any medications to camp

      Family Physician is filled in.

      Parent/Guardian signature.

      Insurance carrier AND policy number.

      Immunization dates are filled in, or immunizations page follows this one.

 

D) A Medical checkup/Health screening (this comes from a health provider):

      Check the date – must be dated after July 31 of the year before last.

      Verify the applicant’s name.

      This is NOT the immunization list – it MUST be a physical exam!

      It MUST have a doctor’s or nurse’s signature.

 

E) Ponderosa Release form (we do not participate in paintball):

      First page completed.

      Both sets of names and signatures and date on the 2nd page.


Please DO NOT FAX these:

  • Your own cover sheet or notes (these sheets fulfill that purpose).
  • Housing Permission Slip (this is for Senior RYLA only).
  • Essays – these are for your use only.
