Rotary Abuse/Harassment Release – for EVERY adult involved in RYLA!

Rotary is required to perform a background check on every adult volunteer involved with RYLA. That means everyone who will be age 18 or over by July 1 must submit the form after January 1.

Don’t worry about remembering – the RYLA Registrar will be more than happy to pester you about getting it done!

Rocky Mountain RYLA pays the cost for this! Using one of the links below, print the form, fill it out and sign it, and do the following depending on your Rotary District (District 5440 covers Loveland, Greeley, Fort Collins and north; District 5450 includes all clubs south of Loveland/Greeley. If in doubt, go with 5450):

• District 5440 – follow the instructions on the form.

Also, when you mail the form, please send an e-mail to Registrar@rmRYLA.org indicating that you have mailed the form so we can track its progress (or we will keep pestering you . . .)
